Application for Admission to the Graduate School

Recommendation Form

Recommendation letters should be submitted electronically through the online application. Please contact Rachel Sligar at with any questions.

 

Virginia In-State Form

Submit this form only if you claim entitlement to Virginia in-state educational privileges pursuant to the Code of Virginia, Section 23-7.4. All forms and supporting documentation must be postmarked by the appropriate application deadline indicated in the box at right. You will be classified as an out-of-state student for admission and tuition purposes if all forms and supporting documentation are not postmarked by the applicable deadline date.

Financial Guarantee and Personal Data for Foreign National Applicants

A certificate of eligibility for the F-1 or J-1 Student Visa cannot be authorized until this document is fully completed and signed. Be sure to include all required affidavits of support and official bank letters, or copy of department award.

Visiting Student

This form is to be used only by graduate students enrolled in the graduate school of an accredited college or university desiring to take graduate courses at the University of Virginia and who intends to return to the graduate school in which he/she is working toward an advanced degree. He/She is required to submit a transcript of his/her credits, and should ask his/her course advisor and the dean of his/her graduate school to complete the following form and return it to the A&S Graduate Admissions Office.

FORMS FOR ENROLLED STUDENTS:

 

Non-Resident-Enrollment Form

This form is to be completed by students applying for enrollment status as a non-resident. Enrollment as a non-resident means that the student will not be using the University of Virginia facilities or faculty time in a manner other than as a non-student. International Students who plan to remain in the US cannot enroll as non-residents and keep their student visa status. Questions regarding eligibility should be directed to Richard Tanson, Foreign Student Advisor, 207 Minor Hall, 982-3017.

Students are responsible for obtaining the signature of their graduate advisor and submitting the form to the Enrolled Student Office along with a non-resident fee, paid by check written out to University of Virginia. Include your student identification number on the check. The fee is $158 per semester for Spring and Fall semesters, due on January 1 for Spring, and September 1 for Fall. The fee for Summer Session is $150, and is due June 1.

Application for Fellowship for Dissertation Research Away from the University

This form is to be completed by students who wish to apply for fellowship money to do research away from the university. This fellowship is limited to payment of tuition for research hours only while the student is away. Students are responsible for obtaining the signature of the Director of Graduate Studies and submitting the form to the Enrolled Student Office.

Reinstatement Form

This form should be used only if the student’s last GSAS enrollment was less than 2 years ago. If the student has not been enrolled in GSAS for 2 or more years, the student must apply through the regular GSAS admission process. This applies to students who withdrew from the University or who discontinued enrollment. It must be signed by the Director of Graduate Studies from the student’s department. The student must sign the form checking “Yes or “No” to any criminal activity that has occurred while away from the University and indicating U.S. military veteran status.

Course Withdrawal Request Form

Late Drop with W/Withdrawing from a Course

If you wish to discontinue a course after the drop deadline, you must submit the Course Withdrawal Request Form. If you miss the drop deadline you have eight weeks from the start of the semester to leave a class using this option. This will result in a W on your transcript. You must talk to the GSAS Assistant Dean for Graduate Programs if withdrawing late from a course will leave you with fewer than 12 credits. This may also jeopardize financial aid or full-time status for purposes of international student visa, NCAA eligibility health insurance, veterans’ benefits, and automobile insurance for “Good Student” discounts. If you stop attending a course without formally dropping or withdrawing from it, you will receive a failing grade. If, after the course withdrawal deadline has passed, you are enrolled in a class you never attended, you will need to petition the GSAS Assistant Dean to withdraw from the class. Failure to do so will result in your receiving a grade of F.

Request for Extension of Time to Complete Degree

This form is to be completed by students who wish to apply for an extension to the normal time limit to complete a Master's or Ph.D. degree. Students are responsible for obtaining the signature of the Director of Graduate Studies and submitting the form to the Enrolled Student Office.

Master Degree Application

This form is to be completed by students applying for the Master of Arts or Master of Science or Master of Fine Arts degree. Signatures of a student's thesis/dissertation advisor, chairman, and department secretary are required by the form. Students are responsible for making sure the form is submitted to the Enrolled Student Office by the appropriate deadline.

Continuation to Ph.D.

This form is to be completed by students applying for a Master's degree who intend to continue on to pursue a Ph.D. Students are responsible for making sure the form is submitted to the Enrolled Student Office by the appropriate deadline for degree application.

Ph.D. Degree Application

This form is to be completed by students applying for the Doctor of Philosophy degree. Signatures of a student's thesis/dissertation advisor, department Chair, and department secretary are required by the form. Final Examination Form

This form is to be completed by a student's department to indicate whether that student has passed or failed final examination of their thesis/dissertation. In cases where a final examination is not required, the form must be submitted by a student's department to indicate that student has completed all requirements for their degree. Students are responsible for making sure the form is submitted to the Enrolled Student Office by the appropriate deadline.

Microfilm Agreement Form

This Microfilm Agreement Form is a requirement and must be completed with a copy of the signed title page and abstract attached and ready to turn into the Enrolled Student Office at the time of inspection of your dissertation. If you cannot download the pdf version of the form available below, you can pick up a hard copy in 438 Cabell Hall.
