Apply to the Mentoring Program

Thank you for your interest in the Foundation Mentoring Program. To realize the full potential of the program it is important that we try to find a good mentor match for you. Foundation Trustees have a wide range of personal, educational and career backgrounds and interests.

To apply to the program:

1. Download and fill out the questionnaire below. This questionnaire will help us identify mentors that potentially will be the best match for you.
2. Fill out the questionnaire by typing directly into the Word document. Feel free to use more than the allotted space if necessary.
3. Upon completion of the questionnaire, please send the Word document to in an e-mail with the subject line "College Foundation Mentoring Program."
